Hourly Habilitation is a service provided by a certified direct care worker to a child or adult with special needs in their home or community setting.  Habilitation is designed to assist the member in acquiring, retaining and improving the self-help, socialization and adaptive skills necessary to reside successfully in their home and community.  Habilitation services include the provision of training in independent living skills or special developmental skills, orientation and mobility training, sensorimotor development, and behavioral management.

Attendant Care is a service provided by a certified direct care worker to a child or adult with special needs in their home or community setting.  Attendant care is designed to assist the member to attain and maintain safe and sanitary living conditions and/or maintain personal cleanliness and activities of daily living. Attendant care also assists the member to remain in their home and/or participate in community activities.  Based on the member’s needs, in accordance with the member’s Individual Support Plan some of the tasks related to attendant care may include: meal preparation and cleanup, eating and assistance with eating, bathing, dressing and grooming, toileting, mobility, transferring, housekeeping and cleaning, laundry, shopping, etc.

Attendant Care

Respite is a service provided by a certified direct care worker to a child or adult with special needs in their home or community setting.  Respite is designed to provide short-term care and supervision consistent with the health needs of the member to supplement care to provide a safe living environment and/or support or relieve caregivers for the benefit of the person.  Respite is to be provided as a planned or unplanned non-routine interval of rest and/or relief to a family member who resides with and provides ongoing care for the member.
